Sending a written request of the export applicant for the issuance of a cargo health certificate or a long-term health certificate. If the applicant is legal, the applicant must submit a letter of introduction on the letterhead of the company with the seal and signature of the managing director.
Submission of a letter of commitment stating that the imported raw materials with protected currency will not be used in the production of the product requested to obtain a health certificate.
Providing a picture of the sanitary permit for product production (manufacturing, contract production, export, supervision ID, etc.) with a valid date.
If you have a shipment or previous export shipments, provide documents related to the export of goods from customs, which include product name, weight, packaging, production date, expiration date, manufacturing series, and brand name, separately for each shipment.
Provide a detailed list of products; By mentioning the production date and expiration date, manufacturing series, invoice number and date, product name, weight, product brand name (in accordance with the health certificate issued) or Packing List for the health certificate of the shipment. For the long-term health certificate, this list must be prepared and submitted for each export shipment.
